Who We Are

Child Health Partnership (previously known as Jefferson Area CHiP) offers support and services to families of children from birth to 6 years of age and pregnant teens and women. Programs are available that build parents' knowledge, confidence, and self-sufficiency in creating healthy homes. For each family, there will be a team comprised of a registered nurse, a Family Support Worker, and the parents. The nurse provides health assessments and connects the family with pediatric and dental care workers, while the Family Support Worker uses the Parents as Teachers program to expand their problem-solving skills.

Child Health Partnership can let you know about agencies that help with needs like food, housing, education, or employment; show you fun activities to do together with your children, and help you find doctors and dentists so your child can stay healthy.

Clients range from birth to 6 years old and their parents or caregivers; some programs for pregnant teens and women. Those who are eligible for Medicaid, FAMIS or FAMIS Pluls or who are unable to get private health insurance.
