Peninsula Agency on Aging (PAA) partners with Hampton VA Medical Center in Hampton, Virginia to prevent the nursing home placement of 'at risk' veterans. PAA provides Options Counseling and Financial Management Services to assist veterans in the self-direction of their care. Options Counseling includes care management programs and services to stay healthy and continue living in your own home and community for as long as possible. Contact PAA for more information about the program that you can share with your VA doctor. Veterans must be referred to the program by their VA Medical Center.
