Evaluation for Intellectual Disabilities Cardinal Care Waiver Services, Langhorne

A program of Horizon Behavioral Health, Evaluation for Intellectual Disabilities Cardinal Care Waiver Services assesses individuals for their eligibility to receive funding assistance from the Intellectual Disabilities Medicaid Waiver.  In order to be eligible for waiver-funded services, an individual must have an intellectual disability, or if under age six, must be at risk for intellectual disability. The individual will be assessed for their level of functioning and must demonstrate the need for waiver services by meeting the specific level of care criteria.
