Who We Are
Danville-Pittsylvania Community Services' Intensive Care Coordination program provides intensive case management to children and adolescents referred by the Family Assessment and Services Team due to being at risk of entering or currently placed in a residential facility. Services include:
- comprehensive strength based assessments;
- identifying appropriate services and implementing plans of care;
- coordinating service delivery;
- evaluating provider quality and providing on-site skills.
