Augusta Health is hoping to help enhance the health of all individuals and families and strengthen the community. Community Outreach tries to find initiatives such as health screenings for those individuals at-risk for multiple chronic diseases, programs designed for those individuals with barriers to accessing healthcare services, and services designed for those individuals who are food insecure.
