Skilled Nursing Care

Fairfax Rehabilitation & Nursing Center offers comprehensive skilled nursing, long term care and rehabilitation programs.  Services in rehab are designed to help residents recover from a recent illness or injury. Skilled care is individually designed to meet resident needs.  Capacity is 200 beds.



Sub-Acute is provided to bridge the transition from hospital to home. Family members are considered important participants in care planning, goal setting and discharge planning. Our Case Management program ensures monitoring of progress and coordination of our Interdisciplinary Team. Has a full time pharmacist, chaplain, beauty and barber shop. Offers in-house activities and has a van for outings. Recommends appointments for admissions.
