Service Coordination, Developmental Disabilities

At Chesterfield County Department of Mental Health Support Services (CCDMHSS) Service Coordination is a service that provides flexible and individualized supports to children and adults with developmental disabilities. The service coordination team helps individuals with developmental disabilities to access other CCDMHSS programs and services for which they qualify, as well as services and programs in the community.

Service Coordination staff provide case management for consumers with developmental disabilities and explain the benefits and resources available through CCDMHSS or community partners. Services include:

• Assessments on current abilities and needs
• Assistance in applying for benefits
• Information and referral
• Informational workshops
• Initial evaluation
• Overview of agency services
• Referrals to outside agencies
• School transition planning
• Short-term case management
