Who We Are
Family Life program provides a place for individuals with intellectual disability to live with families in a private home, rather than in a group home or an institution, maximizing their potential for independent living.
Family Life providers receive training in independent living skills and personal assistance, based on the individual's specific needs. Family Life providers are paid for providing this service. Call David Ballard for information about placing an individual in the program.

Individual must have a diagnosis of Intellectual Disabilities or dually diagnosed with Intellectual Disabilities and Mental Health issues. Customer must be eligible for a Medicaid Waiver.

Referrals accepted from the local Community Services Board (CSB) and their case managers. Contact the Residential Services Associate for more information and to see how to become a sponsored family.
