Skilled Nursing Facility

Norfolk Health and Rehabilitation Center provides intermediate and skilled nursing care, including extensive rehabilitation services.  Capacity is 180 beds.



Our extraordinary team of highly skilled and experienced nurses, therapists, and staff are capable of delivering complex medical services for patients with many conditions including but not limited to:



•    Stroke and neurological conditions

•    Orthopedic surgeries including knee and hip joint replacement

•    Fractures including hip and femur

•    Injuries related to falls

•    Cardiac-related problems including heart failure

•    Pulmonary impairment including pneumonia and COPD

•    Post-surgery recovery

•    Wound care

•    Tracheostomy (uncuffed)
