Skilled Nursing Care

Continuum Home Health Care at UVA offers skilled nursing care. Home health care includes skilled nursing care, speech therapy, occupational therapy and physical therapy, as well as the health care teaching of patients, families and caregivers. Home health care allows patient to remain at home while getting health care prescribed by physicians.



Home health aides are under the authorization of a physician or under the direction of a registered nurse and offer supportive services.



In instances where the patient lives outside of Continuum's ten county direct service area, Continuum will work with the patient to identify and select a Home Health provider where the patient lives to provide the at-home care.
