Who We Are



The Lunenburg Clinic of the Crossroads Community Services Board offers behavioral health case management services. Case Management Services assists individual children, adults and their families with accessing needed medical, psychiatric, social, educational, vocational and other supports essential to meeting basic needs. Services are provided to individuals with behavioral health, substance use and co-occurring disorders. Case managers work closely with individuals in the community to encourage stabilization and recovery. Hospital liaison services are provided by a case manager to help individuals in state psychiatric facilities transition to community living. Case management is available for individuals found Not Guilty by Reason of Insanity (NGRI) at state psychiatric facilities. Services include development and monitoring of conditional release plans with the individual, hospital and the courts. Case management also provides competency restoration services to individuals in the community, including individuals who are incarcerated.
