Options Counseling / Care Management Services

Senior Services of Southeastern Virginia offers adult care coordination in the form of Options Counseling or Care Coordination. Options Counselors help seniors live independently by performing a telephone or home-based assessment and then working with individuals to develop a person-centered plan of care. Options Counseling includes explaining resource options for physical, medical, financial, or emotional needs, coordinating with other agencies as necessary, and providing resources and support to existing caregivers or linking them to the Caregiver Specialist. The Caregiver Specialist provides care coordination to seniors who have caregivers, working with them to obtain the services and supports needed to allow the seniors to remain in their residence of choice and to help alleviate caregiver burden and burnout.
