Who We Are
A program of Horizon Behavioral Health, Community Integration services at the Haley Center assist individuals that are being discharged from an in-patient hospital setting and returning to the Central Virginia community. Individuals are empowered to live successful lives in the community by making informed choices and decisions and participating in the community.
The Haley Center also offers Mental Health Support services which provide opportunities to individuals to maximize their successful living in the community. The individual served and staff work in partnership to achieve and maintain stability through training, assistance and reinforcement of independent living skills, identifying and using resources in the community, assistance with medication management and monitoring of their health, nutrition and physical condition.
Services include group treatment, through case management and mental health supports to promote independence and community living. The majority of services are provided at the consumer's home. Services include medication management, budgeting, and help with activities such as grocery shopping and activities of daily living.
