Case Management, Substance Abuse

The Hampton-Newport News Community Services Board offers adults, who are recovering from substance abuse, help to keep them living independently. Case Managers assist consumers and their involved family members to develop a plan to meet the consumer's needs. Case Managers link consumers to other services, help consumers get services, and arrange needed transportation. Case Managers assist adults who are being discharged from hospitals and residential facilities to resume life in the community. Case Managers provide counseling to support daily living and resolve crisis.
