Who We Are

DC VA Medical Center offers Home-Based Primary Care program (HBPC).  HBPC offers an interdisciplinary team to provide chronic, long-term care to Veterans who have difficulty traveling to appointments.



Provides home based primary care for short term recovery, long term rehabilitation or terminal care.



The HBPC patients have complex medical needs and are unable to come to the medical center for appointments due to their medical conditions. The social worker will meet with you and your caregiver in your home to arrange services that will help you remain living in your home for as long as possible.



The social worker will also help arrange respite care for your caregiver.

Bus Line: Metrobus H2 and H4. Convenient to Brookland-Catholic U. subway stop. Wheelchair accessible.
